I, the undersigned, as the player/parent/guardian of the registered player, hereby:

1. Acknowledge that participation in the soccer development program involves physical activity and potential risks.
2. Confirm that all medical aid details provided are accurate and up to date.
3. Authorize the soccer development program staff to seek emergency medical treatment for my
child if necessary.
4. Accept that the program organizers are not liable for any injuries or loss of property during
training or matches

I (Parent name and surname) being the parent/ guardian of this player, agree to paying the fees as stated above, I absolve Alveda Football Club organisers, coaches, mangers and volunteers from any liability, any liability, loss, damage or injury whatsoever of my child or any effects arising from playing soccer or attending events at Alveda FC, or club fields or from visiting any venue with the soccer club.

Transport - Transport too and from training and matches is parents’ responsibility unless otherwise provided by the club.
I give the player stated here permission to play in this football league pursuant to all the terms and regulations that apply.

If a player decides not to return to the team during the season no refunds will be given, and players clearances will only be issued upon full payment.
